Mary Jo Johnson’s career has been tirelessly devoted to serving seniors and their families since 1988, with a particular passion for dementia care. Her professional passion turned deeply personal when her father was diagnosed with Alzheimer’s disease and lived at an innovative memory care facility she designed and directed. Today, Mary Jo Johnson is the Founder of Aging Concepts, dedicated to supporting aging seniors in our region.

We Provide
Care Management
We evaluate, plan, coordinate, and monitor health services and care needs. We conduct home safety evaluations for you to successfully and safely age in place with the proper supports. We arrange for services in the home. We schedule appointments and assist with pre-registration, navigating patient portals, and insurance payments. We provide transportation and attend appointments with you as your advocate with medical professionals and provide communication and education to families regarding your treatment and management of your healthcare. We arrange for respite care as needed and serve as a caregiver coach.
Life Story
We focus on quality of life by eliminating loneliness and isolation, giving a new sense of purpose. You are the author of your story. An interest assessment is performed and matched with resources. We identify and remove any challenges and barriers. We create a plan by making connections for your successful engagement. It may include involvement in a favorite pastime or introduction to a whole new experience. We focus on "YOU" by having you reflect on yesteryear with a written and/or video graphed Legacy gift created for future generations...a priceless gift for your loved ones. We also have the conversation about choices and what really matters as you plan for today and tomorrow.
Travel Companion
We serve as a companion for local or long-distance travel assuring safety and ease regardless of impairment (mobility, hearing, vision, speech, cognition, anxieties, etc.). We make it possible for you to relocate near family, achieve a bucket list dream, or attend a family event: birth, birthday, graduation, wedding, anniversary, reunion, funeral, etc. Or, you may wish to attend a favorite sporting event or go fishing. You name it and we will try to make it possible.
