Care Management
As your Care Manager, we will evaluate, plan, coordinate, and monitor health services and your traditional care needs.
We arrange for aftercare follow-up from the hospital. We schedule appointments and help with pre-registration, patient portals, and insurance payments.
We provide medication management, transportation & attend your appointments with you. We advocate, educate, and communicate with family.
We arrange respite care as needed and serve as a caregiver coach.

An interest assessment is performed and matched with resources. We identify and remove any challenges and barriers. We create a plan by making connections for your successful engagement.
